What can I expect during my new patient consultation?
Have been suffering for six months or more and seen two or more providers? The initial consultation consists of one-on-one time with the doctor to discuss your problem’s precise details.
The purpose of this visit is to determine if you are a candidate who will benefit from our treatment approach. This requires 15-30 minutes.

What should I bring to my new patient consultation?
Completed forms
Any imaging scans and reports (e.g., MRI, X-ray, MR Arthrogram, etc.)
If submitting to insurance, use the questions on this form to call your insurance carrier, to determine if any reimbursement will be provided by your insurer.

What can I expect if I am a candidate who will benefit from your Movement RX Treatment?
New Patient Exam | The doctor will conduct an orthopedic and functional examination in order to complete a report of findings to establish your patient-centered diagnosis. You and the doctor will review your report of findings and your custom treatment plan. This requires 45 minutes
Treatment Visits | After the examination is completed and report of findings reviewed, you will be scheduled for treatment visits. These treatment visits consist of case updates, functional tests, treatment, and retesting to measure progress. Each treatment visit will be 10-15 minutes.
What should I bring to my new patient exam and any treatment visits?
- Clothing to allow your doctor to evaluate your problem and provide treatment
- Athletic shorts for a low back or leg problem
- Tank top for a shoulder or upper back problem
